Dysfunction in collateral-dependent myocardium. Hibernation or repetitive stunning?
نویسنده
چکیده
Jt is now well established that myocardial dysfunction, once thought to be indicative of irreversible tissue damage, is in many cases reversible. This has led to the concepts of myocardial "stunning"1'2 and "hibernation."3 In both of these conditions, myocardial function is reversibly compromised; in stunning, myocardial function is decreased in response to a previous ischemic insult, and blood flow is normal or nearnormal, whereas in hibernation, function is reduced in response to a continuing flow deficit. However, as reviewed in detail recently,2 in the clinical environment, a sharp distinction between stunning and hibernation may not always be feasible because in a patient with resting flow abnormalities, any increase in demand due to normal daily activities would be expected to lead to ischemia and postischemic dysfunction. Thus, stunning may be superimposed on hibernation. In this issue of Circulation, Vanoverschelde et a14 provide further in-

Hibernation or repetitive stunning – does it matter? The basic perspective
The long-lasting controversy on whether the contractile dysfunction observed in patients with coronary artery disease reflects hibernation – that is, a regulatory adaptation to persistent ischemia – or stunning – that is, an accumulation of reversible damage due to repeated periods of ischemia/reperfusion – is resolved. The perfect adaptation to persistent ischemia as seen experimentally can no...

Ischemic heart failure: pathophysiological aspects Under certain conditions, when viable myocytes are subject to ischemia, prolonged alterations in regional or global LV function may occur and this dysfunction may be completely reversible. This condition may be related to two pathophysiological states: stunned and hibernating myocardium. Stunned myocardium refers to the state of persistent regi...
متن کاملMyocardial Viability by DE-MRI and Low-dose Dobutamine CMR
Introduction Dysfunctional myocardium that remains viable has the potential for contractile recovery after reperfusion. Dysfunctional but viable myocardium has been broadly divided into two closely linked pathophysiological states, myocardial hibernation and stunning. Stunned myocardium is the result of an ischemic insult leading to contractile dysfunction despite adequate reperfusion. Hibernat...
متن کاملMechanisms of chronic regional postischemic dysfunction in humans. New insights from the study of noninfarcted collateral-dependent myocardium.
BACKGROUND Even in the absence of a previous myocardial infarction, patients with coronary artery disease often present with chronic regional wall motion abnormalities that are reversible spontaneously or after coronary revascularization. In these patients, regional dysfunction has been proposed to result either from prolonged postischemic dysfunction (myocardial "stunning") or from adaptation ...
متن کاملReperfusion – the good, the bad, and the ugly
Reperfusion is mandatory to salvage ischemic myocardium from impending infarction. However, reperfusion also causes additional irreversible injury, not only to the myocardium, but also to the coronary microcirculation. Such reperfusion injury is the target of cardioprotective conditioning strategies. Such strategies include brief cycles of ischemia/reperfusion before (preconditioning), during (...
